Team meetings for Team Ohio F.C. will be held on Sunday, June 15 at the Marriott Northwest located at 5605 Blazer Parkway, Dublin, OH 43017. The times are:
- 3:00 – 4:30pm: U8/9, U10, and U11 Teams
- 4:30 - 6:00pm: U12, U13, and U14 Teams
We would like to have each player and at least one parent attend this meeting if possible.
If you have not already updated your online registration and paid your deposit, please do so as soon as possible.

Please note that the registration process is currently open for ages U8 - U14. An email will be sent to players on U15-U18 teams this week as rosters are finalized.After you have updated your online registration and paid your deposit, you will need to complete paperwork which will allow the player to be carded to the team. You will find the necessary forms on our Web site by clicking on “Playing for Team Ohio FC” and then clicking on “Club Forms.” Please print the necessary forms and bring the following items to the team meeting:
- Completed USYSA Membership Form (from Web site)
- Completed US Club Membership Form (from Web site)
- Completed Medical Release Form (from Web site). Form must be notarized.
- Two Color Photos of the Player: 1” x 1” head shot
- Copy of player’s Birth Certificate
- Uniform Order Form (from Web site)
- Uniform payment by check, payable to Soccer Plus
All of these items should be given to your team administrator at the meeting. If your team does not yet have an administrator, one will be selected at the meeting. If you would like to be team administrator for your team, please let your coach know. If more than one person would like the position, the coach will select the team administrator. The team administrator will be responsible for collecting all of the registration materials for the team and will then turn them over to the club manager no later than June 30, 2008.
